What It Detects
The Widal test is a serological agglutination test used to detect antibodies against Salmonella enterica serovars associated with typhoid and paratyphoid fever.
It mainly measures antibodies against:
- O antigen of S. Typhi, associated with the bacterial cell wall;
- H antigen of S. Typhi, associated with flagella;
- AH and BH antigens of S. Paratyphi A and B.
The result is reported as an antibody titre, representing the highest serum dilution at which agglutination remains visible.
Why a Single Widal Test Is Weak Evidence
The major problem is that antibody titres vary widely across populations, especially in typhoid-endemic regions.
A positive result may reflect:
- past infection;
- previous typhoid vaccination;
- background antibodies from repeated exposure;
- cross-reactivity with other infections.
False-negative results may occur:
- early in illness, before sufficient antibodies develop;
- after prior antibiotic treatment;
- in patients with poor antibody responses.
Therefore, a single positive Widal test cannot reliably confirm active typhoid.
Paired Sera and Better Diagnosis
Historically, the test becomes more meaningful when two blood samples taken several days apart show a significant rise in antibody titre.
Even then, culture remains preferable.
Blood culture is the standard confirmatory test during early disease because it directly detects the organism rather than the host antibody response.
Other diagnostic approaches include:
- bone-marrow culture, which may be more sensitive but is invasive;
- molecular methods such as PCR in specialised settings.
In endemic regions, interpretation of Widal titres should also consider local baseline antibody levels, because a titre considered high in one population may be common in another.
Public-Health Significance
Widal testing remains common in many low-resource settings because it is:
- inexpensive;
- simple;
- rapidly available;
- less infrastructure-intensive than culture.
Its continued overuse, however, has an important antimicrobial-resistance implication.
A false-positive test may lead to unnecessary antibiotic treatment, while a false-negative test may delay appropriate therapy.
This can contribute to:
- inappropriate use of cephalosporins or azithromycin;
- selection of resistant Salmonella strains;
- distortion of typhoid surveillance data.
For this reason, typhoid control increasingly depends on better microbiological diagnosis, antimicrobial-susceptibility testing and vaccination, rather than routine reliance on a single Widal result.
Core Point
The Widal test detects antibodies, not the Salmonella organism itself.
Its value is therefore limited by background immunity and cross-reactivity, and it should be interpreted only alongside clinical findings, local titres and preferably culture-based evidence.
