Classification and Disease Profile
Bundibugyo virus (BDBV) is a species of the genus Ebolavirus, family Filoviridae, and is one of the viruses that causes Ebola Virus Disease (EVD) in humans.
It was first identified during an Ebola outbreak in Bundibugyo district, western Uganda, in 2007.
Like other ebolaviruses, it is an enveloped, negative-sense single-stranded RNA virus.
Human infection can produce:
- fever and profound weakness;
- headache and myalgia;
- vomiting and diarrhoea;
- abdominal symptoms;
- rash;
- haemorrhagic manifestations in severe cases;
- multi-organ dysfunction.
Its case-fatality rate has generally been lower than that associated with Zaire ebolavirus, though Bundibugyo virus disease can still be severe and fatal.
Transmission and Outbreak Pattern
The natural reservoir has not been conclusively established, but fruit bats are considered likely reservoir hosts for ebolaviruses.
Transmission occurs through:
- contact with infected wild animals;
- direct contact with blood or body fluids of an infected person;
- contaminated medical equipment or surfaces;
- unsafe handling of bodies during funerals.
Important recognised outbreaks include:
- Uganda, 2007–08, where the virus was first identified;
- Democratic Republic of the Congo, 2012, where Bundibugyo virus caused another Ebola outbreak.
The incubation period falls within the broader Ebola range of roughly 2–21 days.
Public-Health Significance
Diagnosis requires specialised laboratory methods such as RT-PCR because symptoms overlap with malaria, typhoid, other viral haemorrhagic fevers and several tropical infections.
Control depends on:
- rapid isolation of suspected cases;
- contact tracing;
- infection-prevention and control;
- safe burial practices;
- surveillance and laboratory confirmation;
- community engagement.
A major limitation is that the widely used Ervebo vaccine targets Zaire ebolavirus and is not specifically licensed for Bundibugyo virus. Likewise, the monoclonal-antibody treatments authorised for Zaire ebolavirus cannot simply be assumed to work against Bundibugyo virus.
Bundibugyo virus is therefore important because it demonstrates that Ebola is not caused by a single virus. Different ebolavirus species vary in geography, case fatality, vaccine coverage and therapeutic options, making species-level identification essential during outbreaks.
